Patient Form
Prior to your first visit to Skocik Chiropractic, you’ll be asked to fill out patient forms in the New Patient Center. This paperwork covers your medical health history and information about your current symptoms. If you can't complete the paperwork online, it's not a problem... please come to your appointment 15 minutes early to complete the paperwork in our office. On your first visit, please bring your identification card, insurance card if applicable and a medication list.
Consultation
Then, you’ll have a one-on-one consultation with Dr. Albert Skocik. He’ll talk to you about your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Albert Skocik to learn more about you, to determine how chiropractic care can meet your goals.
Examination
After your consultation, Dr. Albert Skocik will perform a complete chiropractic examination. Dr. Albert Skocik will also perform standard neurological, orthopedic, postural, and physical tests. However, nothing will be done in our office without your consent. Once Dr. Skocik has all of the information, he’ll render an accurate diagnosis. He’ll also let you know how chiropractic care can help you get on the path to recovery. Dr. Skocik has years of experience helping patients recover from auto accident injuries, work injuries, slip and fall, sports injuries and more. He has successfully treated many types of injuries, including whiplash, soft tissue injuries, herniated disc, sciatica, bulging disc and degenerative disc disease.
X-Ray Studies
Depending on your specific condition, he may take x-rays as well. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their x-rays up close. In some cases, they can even identify their misalignments and degeneration themselves.
Treatment
At the conclusion of this initial appointment, you can choose to experience your first chiropractic therapy treatment. Once your treatment is complete, you will stop at the front desk to schedule your Report of Findings appointment. Our front desk will give you a copy of Dr Skocik's book, "Years to Life, Life to Years."
Dr. Skocik will review your information and finding, then develop a treatment plan just for you. On your second visit to the office, you will receive a report of findings where Dr. Skocik will go over your treatment plan with you, then you will receive your adjustment and therapies. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.
